Frequently asked questions

01

What does Gesund do?

Gesund interprets medical evidence across sources and time to reconstruct the applicant’s clinical story.

It establishes what the record supports about each condition: when it began, its severity and control, how it has changed, how treatment is working, and how it relates to other conditions.

The result is a clinical story and structured findings, with supporting evidence, data gaps, and unresolved questions made clear.

Clinical records may also be obtained through health information exchanges (HIEs). Gesund interprets the available evidence alongside the other information in the file.

02

Do all medical record requests return the same information?

No. “EHR” and “APS” do not describe a uniform set of information. Different access methods can return different portions of the same person’s medical history, overlapping records, or duplicate copies of the same information. More data does not necessarily mean a more complete clinical picture.

A patient portal, connected application, health information exchange, or record-retrieval service may return different combinations of clinical notes, medications, labs, procedures, and encounter history. An APS also varies according to the request and what the responding practice supplies.

What matters is which providers and time periods are represented, what clinical detail is included, and what is missing. A record may contain lab results without the specialist’s interpretation, or a diagnosis without enough history to establish its severity or control. A lengthy file may still leave important questions unanswered.

Gesund interprets the evidence available across sources and time, showing what supports the clinical story, where the data has gaps, and what remains unresolved. Information absent from the supplied record is not assumed to be absent from the applicant’s history.

03

Does Gesund use AI?

Yes. AI is primarily used in development and design. It does not perform the clinical interpretation of the applicant’s record.

Gesund’s clinical logic is defined by clinical and insurance medicine expertise. The output shows what supports the interpretation, what limits it, and what remains unresolved.

04

How is clinical interpretation different from medical record summarization?

A summary can organize findings by condition, build a timeline, and make a record easier to review. Clinical interpretation goes further by establishing what those findings mean together.

Gesund follows each condition across sources and time to characterize its severity, control, progression, treatment response, and relationships to other conditions. It considers evidence that supports the interpretation and evidence that counts against it.

Two records can mention the same diagnosis and describe very different clinical situations. Interpretation establishes what the evidence supports in each. Where a conclusion cannot be supported, Gesund abstains.

05

What records and data does Gesund read?

Prescription history, medical claims, laboratory data, exams and paramedical evidence, electronic health records (EHRs), and attending physician statements (APS). That includes structured data, clinical notes, and scanned or faxed documents such as PDFs, TIFFs, and photocopies.

Gesund interprets evidence regardless of which vendor obtained it. You can keep your existing suppliers, and Gesund evaluates what they return alongside the other evidence in the file.

A medication change, a lab trend, and a specialist’s note can all contribute to understanding the same condition.

06

Does Gesund score, rate, or decide the case?

No. Gesund provides clinical interpretation. It does not assign risk scores or ratings, set pricing, apply your risk appetite, or make the underwriting decision. Those decisions remain with the carrier or reinsurer.

The same interpretation is delivered as an underwriter-ready clinical story and structured condition-level data for rules, analytics, and data science teams. Your team applies its own guidelines and judgment.

07

What is an APS, and why is it hard to interpret?

An attending physician statement is the medical record requested from an applicant’s treating physician. It may include clinical notes, specialist reports, medications, and lab results, often delivered as PDFs, scans, or faxed pages.

The challenge goes beyond length and format. Evidence about one condition may be spread across years of visits, mixed with duplicate information, or complicated by conflicting findings. The record was written for clinical care, so it may not directly answer underwriting’s questions.

Gesund interprets that evidence across the record, showing what it supports and what remains unresolved.

08

What happens when the record does not support a conclusion?

Gesund makes that uncertainty explicit. It shows where evidence is missing, incomplete, or conflicting and what remains unresolved. Where a conclusion cannot be supported, Gesund abstains.

Insufficient evidence is different from no findings. A gap in the record is not treated as confirmation that a condition is absent or well controlled.
